And what the others say here is the interpersonal debt deficits is a defined area when the problem is not occurring. In the first three categories The others here talk about IPT is having three phases. The initial phase is usually one to three sessions. And this includes an evaluation where they are looking at what is happening with the client and any other conditions that may be going on. And during this time and inter-personal inventory is conducted This is a really important part of the whole model itself that needs to be done right away in the initial phase. Because what’s going on here is that you’re looking at patterns in current and past relationships that may be occurring. So in this first of three sessions what you’re trying to do is understand what the problem is and again this is the medical model. So they’re talking about diagnosis such as depression Alenia panic disorder PTSD And then any other conditions that may be going on that are affecting the client further and then developing this inner personal inventory where you’re looking at the client’s past relationships as well as their current. Another part of this first of three sessions is you want to provide a case formalization. Our Conceptualization. And what this does is it looks at defines the diagnosis within the medical model. And what you’re doing here according to the theory is you’re providing The client with a transitional quotes sick role. And what this does according to the researchers and authors of this model is this the VBA is they state here. Any responsibility on the client’s part for their difficulties. So providing a diagnosis can help clients feel like there’s not something wrong with them. There’s just something wrong And that’s a really important. Oftentimes you talk about not using diagnoses because they they label people. And I’ve seen it both ways. We’re a diagnosis can have a stigma to it it can be labeling. And that can be questionable. On the other hand I’ve had clients who are relieved to know what is wrong. Oh it’s that it’s not me then especially someone with depression. Who stinking has been distorted by the depression itself. And they may begin to feel they’re lazy or they’re stupid or they’re undisciplined. When in fact they’re struggling with an illness. That is preventing them from being able to function as they would like to function. And when they find out oh it’s not me. It’s this thing that’s making me sick right now And it buy treat if I can get better it can be a big relief to clients. When you provide what let me get back your skis me that when you link the diagnosis to an interpersonal problem then this also help she look at how the diagnosis is effecting the problem and the problem is affecting the diagnosis. In other words if you can better your relationship and the way you’re interacting with people You can alleviate symptoms at least many of the symptoms of the diagnosis. Then the other part of the one to three sessions that you want to focus on is the treatment plan. And you want to work with a client to agree on a treatment plan. So this is a very important part of the initial phase. Now the case formulation or the case conceptualization you talk about that before Provides is the author state here the inner personal problem focus. The which the pre-check pro proposed change mechanisms or activate it. So let’s think about that. What that means. When you provide a focus to the client about what is wrong and why things are happening the way they are then you are also looking at what needs to be changed. In order to get better So when you’re conceptualizing a case from the IPTV point you’re looking at an interpersonal problem. Looking at the problem the client’s symptoms through the lens of an interpersonal conflict. And one of those categories that we just discussed either because they’ve lost somebody. It’s because they have a role conflict. It’s because they have a bro dispute. Are social deficits When you look at their depression symptoms or their eating disorder symptoms or their anxiety symptoms through that lens of interpersonal problem. Then this means that the change that needs to be made is within the relationship. And once those changes are made and the way that they relate to others. Then the symptoms will be alleviated. The others talk that the middle phase here sessions four through nine are up to 13 And this is the main area of work or you’re a dressing the interpersonal problem in order to reduce symptoms. And then the final phase of the three final sessions is a direct discussion as the authors state here of termination. And going over the improvement the client is made what kinds of successes they’ve had and to help them maintain. By Dressing possible future problems and how they can work around those. That’s all I’m going to go over for tonight’s lecture. I’m going to talk a little bit more about this article and the upcoming weeks. But I wanted you to know what IPT is. And to have a better understanding of some of the terminology because it can be a little bit complicated initially. When you start to learn about it have a good week. Hi students I’m going to continue the lecture an interpersonal therapy and this podcast. And I’m still using the article by Lipset and Markowitz And we’re on page 1140 and I’m talking about here the change mechanisms that the authors discuss. The first change mechanism is enhancing social support. And normally we think about social support is being a concrete surface. We also consider social support human connection. And these certainly are social supports for our clients and for ourselves that are important The authors here carry this concept of social support a little bit further than they do discuss it as the availability of people in our lives that can help us either through providing us with physical resources are with physical touch our care and concern. And also people that we feel attached to is Bowlby called the term Attachment. And stated that we we had an ongoing need for this With IPT what they’re talking about is even more. Then these kinds of things such as infant mother bonding. While those are important IPT focuses down all aspects as it says here. I’m going to all aspects of the current relational context including the individual’s functional roles in society. And so this is really important when we think about enhancing social support with our clients As a change mechanism through this this approach of IPT. So some theorists have outlined as it says here the benefits of social support and how these benefits can positively affect mental hell. Some of the benefits they discussed here are social influences on health behaviors such as exercise or eating well are getting enough sleep. Also positive peer pressure Which king can encourage people to do things to to go out with them and to be with them. And this can generate a positive emotional state. And two other examples they discuss or interpersonal emotional emotion regulation and social roles. And one of the things the authors point out here is that in emotional disregulation and I’m quoting here from the article emotional dysregulation is a feature of many psychiatric disorders. So Looking at depression believe me bipolar disorder anxiety emotional dysregulation is inherent know those disorders. And a lot of the recent research looks at cognitive ways to regulate your emotions through your thoughts and that this is an internal process. And so when IPT look set emotional Regulation it takes it out of that internal perspective and looks at it within relational systems. And to Authors Talk about how people can in our lives can help us regulate our emotions through helping us reappraise situations that are bothering us or through containing our anxiety. Being a stabilization. Having empathy providing a warm hug And that through relational experiences our emotions can become regulated rather than relying on just the internal cognitive regulations. They also go on to talk about how relational theory suggests that I’m going to quote here under positive conditions the developing child gradually internalizes the function at the caregiver. However adults continue to rely on loved ones for this holding function. So even though we may have grown up with a pause of attachment and we were able to internalize that ability to soothe ourselves in some conditions. Still with adults we want a loved one to share our experiences with both positive and negative. The second area that they discussed was social roles. And what they talk about here is having a social role helps regulate emotions because we have obligations. People expect things of us. We have places that we have to be to show up at if you’re involved in a church and you’re leading a church group you have to show up there. If you’re involved in a choir Community choir then you’re expected to be at rehearsals. So there’s this there’s this regulation of emotion that occurs through social support. That is the result of our own roles are and social roles and expectations that come with those roles. So here the authors talk about how these social roles can help stabilize our mood states and also give us a sense of meaning and purpose as they talk about here That it makes a difference in a person’s life when they feel needed. So when we talk about enhancing social support is a change mechanism. In IPT I want you to think a bit beyond just resources in connecting clients to resources but helping them look at and establish social roles. Helping them to look at how the The emotional reaction can be battered when they are with supportive people. Now some typos B and relationships can enhance or exacerbate negative conditions. So we want to help clients create positive social supports. And look at how they can use those social supports to feel better in their lives The other side of the page here they talk about enhancing social support in IPT. And in resolving interpersonal problems. Enhancing social support in IPT differs from supportive therapy. And this is an important thing that I want you to remember is that IPT is not supported therapy. It is a dynamic affectively charged therapy. We’ll talk about that in a little bit. So it goes beyond what we call as supportive therapy which really is a more general way of improving support. It also is different than a psychoanalytic theory. We’re. The psychoanalytic theories tend to see the therapist as the holder of the client’s anxiety that container And the provider the emotional connection and hopes that the client would generalize that outside of the therapy room. But IPTV really while it does see the therapist as having an important role. It is very ensuring and holds a lot of emphasis on going beyond the therapy room and really deepening relationships outside of the context of therapy So the back on the film that you watched April and how Paul continually encouraged her to connect with people outside of the therapy room. I’m going to stop here and do each of the change mechanisms in a separate podcast. Hey students in our last podcast and the interpersonal therapy model we talked about change mechanism one which was enhancing social support. In this podcast I’m going to be talking about change mechanism to which is decreasing interpersonal stress. And I want to point out just a few things here that the authors talk about. First they talk about the effects of negative interpersonal stress is going beyond a lack of social support. So while the first change mechanism enhancing social support It’s one way to help decrease interpersonal stress. The effects of negative inner personal experiences really go much beyond. Just a lack of social support. Ipt talks about the focal problems is major life events are the chronic stressful conditions as the author state here so that means thinking about the boar domains that they discussed in the article earlier which are the complicated agree. The role transitions that are major life events roads speeds which can be chronic and stressful. And the interpersonal deficits. These vocal problems have been empirically linked to depressive episodes and also other psychiatric disorders. And they go on to state that although not all prominent stressors or in a personal most are and then they discuss in personal stressors and that impersonal stressors can also have meaningful effects on the interpersonal domain. For example if a client were to lose the job which is considered an impersonal stressor are to have an illness physical illness. These can cause significant stress. And what happens is if the person loses their job they have a decrease in finances. We can all imagine how that would be stressful. Or if they have Physical help compromise they may not be able to do the things that they were able to do in the past things like cleaning the house to bike riding to hiking to enjoying their life physically. So these kinds of impersonal stressors occur as a result of a job loss or an illness. However as your author stake your stress is increased. When these events undermining the individual social role and interfere with their relationships In other words these are interpersonal consequences that become the focus of work and IPT. So you have a job loss. It affects your finances which is stressful but that stresses also increase because your social role is now changed who you were in your career. Your title is now gone. In addition that create stress at home because not only do the bindings is it that you in your relationship and your ability to do the things that might have Helped you maintain Joey and been in your life. You also have this diminished sense of meaningfulness. So even the impersonal stressors can really create a lot of consequences that become the work that Boko work by PT which is those four areas that we discussed before. I want to talk a minute about minor events here the authors talk about daily hassles. I’m sorry the screen is not cooperative. So some of the daily apples at hassles that you might go through. That. Bees also have an effect on the major stressors. And then you also have chronic adversities. Were the Earth enduring stressors such as single pair to family conflict. So these daily hassles as well as chronic adversities also are part of the stressors that can result in issues within those four domains. Go up here to the top. The authors at dress the interpersonal stress within each of the interpersonal problem areas in this section. So they talk about how the primary goal here is to reduce the patient stress experienced in the context of the interpersonal problem And they believe that when you do that it diminishes decreases our eliminates the symptom. For example the symptom of depression or the symptom of anxiety. When you reduce the stress within the context of the person’s relationship then this diminishes the symptoms. So let’s take the first inner personal area that we’ve talked about. One of the major focuses of IPT is great. And this is of course considered one of the Stressful life events is losing a loved one through debt. And what I P T does is it works to facilitate the grading process and help the patient re-engage and other relationships. So here you’re actually helping the person grieve. This is where the emotional processing comes in. You’re helping the person base their loss helping them process their emotions so that they can come to a place of acceptance of it. And then in addition to that you’re helping them reengage in other relationships so that the loss is not gone ever are forgotten. But it becomes less distressing because there’s other meaningful relationships that the person can become engaged in. For Ronald to speeds what you’re trying to do with IPT here is decrease the stress that happens with the ongoing friction in a relationship or the anger. Shame is they discuss here. The authors go on to talk about helplessness and alienation. Did these kinds of feelings. Anger shame helplessness alienation. And the ongoing friction can occur and conflictual relationships. And so here what you’re trying to do is less than the stress associated with these things. So that you’re not stressed. When anger recursive relationship. You don’t experience shame or it’s at least decreased What what happens when you’re working with IPT and this error yet is sometimes the complet gets worse before it gets better. And the reason for that is that when we help people communicate their feelings more openly and honestly rather than a passive aggressive way which could be friction that occurs or create shame or alienation but us them and teach them to communicate better. And an open and honest way this can increase some of that conflict You have to prepare the client for that and let them make that choice about whether they want to be assertive or set a boundary. Knowing that there could be a consequence for it and help them anticipate but that might be and also how they might be able to handle back cope with it. But this allows the relationship to have options. So let’s say it’s a couple and they fight a passive aggressive way and they go to bed angry. And they both feel shame and they both feel helpless and resolving The problem and there’s always this ongoing insidious negative energy. When they start to talk openly about these feelings and start to express themselves more openly. They may not like hearing what the other person has to say. And this can be stressful. So we want to help people cope with that. It also may cause them to renegotiate the relationship so they could renegotiate it in a way that says okay when we get angry here’s what we’re going to do. Gonna take a 10-minute break. We’re going to do this. I’m going to do this And then we’re going to come back together and try to spend five minutes each talking about it. Or they may renegotiate the relationship and decide that they don’t want to be in a relationship anymore with role transitions. What’s happening here is there’s a strain that occurs on the person’s ability to adapt or the way in which they adapt. And so When the person is strained this of course creates stress. And this can occur in happy positive situations such as marriage. And anyone who’s ever planned a wedding knows that it can be extremely stressful. You cannot please everybody and your own excitement about your own day. Can not be exactly what you thought it was going to be the birth of a baby even if its a wonderful exciting time. It can also be stressful because The couple that was free to do things that they wanted to do all the time is not free anymore. Started graduate school that can be stressful. I’m sure you own and that are getting a promotion and having this new opportunity is gray and having more money perhaps as the grade. But it’s still stressful because there’s also expectations that go along with that. So what IPT doesn’t in this situation with straw role transitions is that they try to help the client reduce the stress at the transition First by acknowledging and mourning the losses. So a loss in the birth of the baby is the loss of your freedom. To do whatever you wanted did mothers know this because they lose their freedom at the moment of conception because they have to change the way they eat. We have to change the way they fund that they had. Maybe they used to drink a glass of wine every night they have to change that. They also have to in some cases changed the way they get enough sleep because they’re not able to sleep in later months of pregnancy. So they have a whole hormonal Change they have to mourn the loss of their body. And the way it used to be Youth and aging. So helping the person acknowledge and mourn these losses is very important even if it’s a positive event could also be true for marriage or for starting college. Some of you may have been in full-time jobs before where you would charge a lot of things and you’ve had to give up that since privilege really to go back to college and become a student and it’s a role That were used to be the leader now you’re more of the follower. So this can also create stress. And so IPT wants to help the client reduce that by first acknowledging and mourning the losses than clarifying the positive and negative aspects of this change or this transition. And then identifying and processing any strong feelings about the transition. And then modifying the interpersonal patterns. So okay so now we have a baby. So our relationship now is going to change We’re going to have a different way of relating. We’re not going to get as much sleep. We’re not going to be able to have sex is often where I’m not going to be able to go out to eat and be free to stay out after work light. These are things that we have to mourn then acknowledge and clarify what’s positive about it. What’s negative about it than identifying process and a very strong feelings. And then changed the way we relate. And then an inner personal deficits you’re decreasing the stress. If loneliness isolation And loneliness and isolation and this becomes the main goal. So here the main goal when you’re working with in this domain of inter-personal deficits is to help the person bill less lonely unless isolated. The authors talk about Horowitz his work and the Horwitz proposed two categories of inter-personal stressors that involve different interpersonal needs. And I think these are interesting. One is related to communion. Communion with others And what happens is when you have a stable attachment that’s interrupted either through lat death or maybe divorce and you thought it was staple in it’s no longer stable or it was stable in some ways. This can lead the person to feel very lonely and rejected and disconnected. And so you might look at this as a role transition like a romantic breakup or maybe you’re not a mom anymore. Maybe your your child has grown up. And sometimes if adolescence has been difficult You’re really excited when they leave but that lasts for about five or ten minutes. Or maybe you’ve had to relocate and you’ve really lost a set of friends that you had developed over years. And so you can start to fill this loneliness our rejection or a disconnection. And then Horwitz talks about the other type of stressors are stressors where there’s a challenge in mastery. And this can make the person feel helpless or in barrier or like a failure. And this is more along the lines of unemployment or illness. We’re not able to do the things that used to be able to do. So I think that’s an interesting way to also look at the interpersonal stressors and and how you would help decrease those stressors that helping people have meaningful lives and also helping people reconnect and being communion with others. In decreasing interpersonal stress There’s lots of they talk about her psychological interventions that reduce stress by looking at changing cognitive processing. And all of these have the common goal of helping the client better manage and cope with challenging situations. Ipt however abuse the interpersonal problem itself. Is the primary culprit. Sure author stake here for current feelings of stress. So we’re CBT may look at changing cognitive Processes thoughts are mindfulness may look at changing your attention to the present. Are applied relaxation may look at. Decreasing stress that the three relaxation IPT looks at the interpersonal problem. Is the reason you’re feeling stressed. And what it does is seeks to decrease the stress by changing this dress full aspects. I’m quoting here of this reality or the patient’s relationship to it. So you’re seeking to decrease the stress by changing this stressful aspects of the reality of this inter-personal relationship or your relationship to the reality of this relationship. So what you’re looking at also is how this takes a medical model approach just sick role. As well as the therapeutic relationship as means of decreasing stress So the therapeutic relationship can temporarily alleviate social burdens. But remember in this therapy. So like psychoanalytic where you want the client to the hab and dependency. And I don’t mean the same thing is fostering dependency. But necessary dependency to work out their issues we cue and then generalize them out into the world. What you’re talking about here is a very temporary alleviating the social burden through the therapist role As someone who was essential support but in this model you quickly start enhancing other social supports. And then also temporarily alleviating expectations and social roles said you know if you get sick and you have the flu everybody goes well you can’t do your normal chore of going to the grocery store when you have a fever When bad lay down and take care of yourself I’ll go to the store for you. So the social role whatever your role was if it was to be the shopper and go get the staff and cook the meal is alleviated with the sick role. Well we don’t do that when we have emotional or psychological illnesses I’m saying that in quote. So somebody’s depressed we don’t say Well of course you’re depressed and that’s why you feeling blue and it’s hard for you to get started today or it’s hard for you to find the motivation to go for a walk And I understand that that’s hard for you. How can I help instead we get angry at that person because they’re not doing what we want to do. So this is talking about looking at depression and the other kinds of things anxiety and eating disorders. As you need help. You need support. And you need to understand a that does it mean you get to have the flu forever. No one wants to have the flu forever. And being depressed or having an eating disorder having extreme anxiety is not enjoyable either. So what we want to do is decrease stress by alleviating the social burdens which is using the therapeutic relationship to be a support system but to quickly move the person into other support systems. And then we alleviate the expectations you should be doing what you always do when you’re depressed. No you should feel bad. You should have a hard time getting up to go to work when you’re depressed. How can we help you feel better so that you can do those things. I’m going to stop here on this change mechanism And this podcast was how to Yale. Hi students this week we are continuing to discuss interpersonal. Both date. If you think about it How do we relate to our inter-personal relationship we experience love. Anger frustration. Contentment anxiety feelings of abandonment. All these kinds of emotions come up in the context of relationships. So when your authors talk about this they talk about we relationships are really operating within an emotional system. So it makes sense that we would include the processing of emotions and our therapy with our clients. The authors also talk about central task in inter-personal therapy involved confronting and surmounting. The problems within the relationship by identifying processing and expressing emotions. So it’s important when you’re using IPT. You help the client identify their emotions process them and express them as they relate to the interpersonal context. The issue that they’re bringing TA. We don’t always know what we’re feeling. We may think we’re filling angry but we’re really feeling hurt. We may think that we’re feeling anxiety. But it’s mostly fear a fear of abandon that. So we want to look at how we would help clients identify their feelings and emotions and then process them. And when I say process they don’t talk about them on a logical level but to feel them and experience them on an emotional level while they are intellectually processing there. So as they are experiencing the feeling being able to talk about that feeling and why it feels painful or uncomfortable. And then expressing their emotions being able to have the opportunity to get their emotions now. To have that cathartic effect So I want you to think about the importance of emotions in relationships. And that our job when we’re using IPT is to help clients identify processed and express emotions within the context of the interpersonal problem that they’re bringing to you. So here over here in this section I want you to look at how The authors talk about IPT is really quickly shifting the expression and awareness of emotions from this session to the real relational context. So while we are emphasizing the processing of emotions emotional work involves understanding the interpersonal challenges. Interrelationship and being able to adapt to them reacting to interpersonal stress a overcoming complex. So we want to make sure that we take the expression and awareness of emotions that we help the client get in touch with from that session two this context where they can meet the interpersonal challenge. And under steam what their emotions are related to that challenge where they can react to the stress that occurs in a healthy way. And where they can work on overcoming complex So IPT really focuses on fixing the problem in the interpersonal context rather than the underlying problem the patient has a processing emotions. So we’re not going back to childhood. And dealing with our anger at our mother. Though that might come up in a session. My mother abandoned me a client might say. And I feel like I can’t trust this person because therapy and to me. Ok let’s process those feelings of abandonment Let’s look at how you can know when they’re about to come up. What happens in your body do you tense up to clench your fist you clench your jaw do you start to feel a rapid heart rate what emotion are you experiencing right now the fear what is the fear related to the abandonment and then being able to say you know separate what is from the past and what is happening currently Be able to understand these emotions are occurring because I feel threatened in this relationship that he may leave or she mainly. Are they mainly and this brings up a lot of anxiety and fear for me of abandonment. And I know that’s what’s happening right now. So I’m going to take some deep breaths. I’m going to slow down and not react. And then I’m going to work on expressing my emotions in a calm manner. So what we’re doing is staying focused on the problem in the interpersonal context. And taking that expression and awareness of emotion that occurred last session and putting that back into the real relational context. So I want you to really think about the difference between processing emotions and intellectually. Talking about emotion. One involves feeling. And thinking processing involves feeling. And thinking. Not just thinking mechanism for improving interpersonal skills. And here what we want to look at is how to help clients improve interpersonal skills within the context of their interpersonal problems. So a lot of times therapies make improving interpersonal skills of primary goal. Sure authors talk about here. Here there what they’re referring to as skills training programs. And that’s not what we’re talking about when we talked about improving interpersonal skills with IPT. It’s not that this isn’t helpful. It it can be very helpful. But what we’re looking to do is to really improve interpersonal functioning within the context of the interpersonal problem that the client brings to us. So let’s go back to those problem areas. For example robe to speed. Again we’re trying to improve interpersonal skills primarily within the framework of the interpersonal problem area. Does it mean that you would never use skills training is that that’s a fine thing to use. And almost all therapies deal with improving ways that we interact with others and communicate with others. But in IPT what’s different is we’re actually looking at it within the context of the problem the client’s presents rather than generally. So if the client has a role to SPY We want to help them learn to communicate feelings. More directly with the person that they have this dispute with your authors talk about here. You want to help the client use constructive assertiveness and also learn to diffuse tension. So let’s say roll to speed is occurring between a client their boss. When the worker the client doesn’t communicate directly. This can be frustrating for the boss or causes anxiety. Be irritating because false is very busy and they need the work or just to spit it out. So helping this client learn to communicate your feelings more directly. Also using constructive assertiveness being able to assert themselves in situations. When they didn’t need to communicate feelings to be able to learn to use I statements to let someone know that there’s a problem up front rather than the reactionary being proactive about things. And then also learning to diffuse tension. How can they help the person not feel tense when they’re talking to them. Enroll transitions. Ipt focuses on skills that are needed to adapt to the new interpersonal ball. Okay so if we’re talking about somebody who has an illness this may mean that they need to set limits for the caring person in their life but who’s being intrusive. So they need to be able to express their needs about setting limits. If the person is a retired person they may need to learn to initiate social contacts and activities. If a person’s whole life has been work and that’s where they’ve gotten all their meaning. They’ve led a lot of these areas social contexts Sexual activities go by the wayside and they’re going to have to learn how to initiate these things and determine how to communicate their needs in this area. And of course looking at great grief we’re looking at when you lose someone who has functions within the relationship. And now the other person has to take on those functions. I’m not just talking about task. I’m talking about interpersonal skills that include independent functions like calling the plumber. Yes that’s a task. But if one person is more ease of dealing with those things and is always taking care of it and is more outgoing and is able to talk about those issues more easily. Then it’s easier for that person to do it but when they passed away they were no longer able to be in that role because the severe illness than the other person has to learn how to develop the interpersonal skills to be able to carry out these functions And then finally we want to look at communication skills in terms of interpersonal professional interpersonal deficits. There’s a lot of skills that may be needed to overcome social isolation and that may actually involve skills training. Also looking at how to communicate one’s needs. So all of these concepts right here talk about addressing inter-personal skills within the framework of the problem area. As you read your article and you watch the two films that an April. I want you to think about processing emotions and improving interpersonal skills within the context of the interpersonal problem. When you watch the two films of April once you to really focus on those things. And you have a great week.
