Healthcare is constantly changing due to evidence-based practice that would allow for more safety benefits to the patients, family, and healthcare professionals. This isn’t any different when it comes to surgery. Surgery has changed over the year to minimize the risk to the patient by limiting the number of open surgeries being done to conducting minimally invasive surgeries. We know that open surgery is where the surgeon cuts through the skin and tissue to allow for a full view of the structures or organs involved (Surgical Care, n.d.). Then minimally invasive surgeries are done using a device like a robotic machine to minimize open wounds, which will help decrease risks for patients.

Because of the need to minimize risk to patients and to minimize fatigue from long open surgeries a robotic machine has been created research and development-based innovation known as the da Vinci subgingival system. The da Vinci System is a robotic device used by the surgeon where it translates the surgeon’s hand movements at the console in real-time, by bending and rotating the instruments, by using highly magnified 3D high-definition views of the surgical area (Da Vinci System, n.d.). Instead of a surgeon cutting you for open surgery, this machine will only require 1-3 small incisions. The machine does require to have 3 different parts it the first is the surgeon console where the instruments are controlled, then the patient cart where the cameras and instruments are, and then the vision cart where it will display real-time images to the surgeon to have a better view.

The robotic machine allows for certain benefits for the patient that allows the machine to not only be research and development innovation but also customer based. The da Vinci System allows for a faster recovery for the patient, while reducing pain, decreases the risk of infection and complications, less blood loss, shorter hospital stays, and less scarring due to smaller incisions (Robotic surgery, n.d.). The da Vinci System is a great machine to use for patients with other risks involved due to illness. Also, due to a smaller incision, there is less chance of the patient losing a large amount of blood, which decreases the need for blood transfusions. Just like with any surgery there are still risks involved and when patients need surgery it is important to ask the surgeon what the risks are and how to minimize those risks.

A health care delivery issue that we encounter in the pediatric in-patient unit, is starting IV’s. Our patients either don’t have a central line already, or they need the IV for fluids and/or antibiotic therapy. The issue is having to puncture or “stick” the child more than once because either the patient is moving too much, the vein blows, or we just cannot find a good vein to use. The parents get upset when their child must be poked several times in order to find the perfect vein. Not to mention how upset the child is when they are screaming and yelling in your face as you and your co-workers are trying to hit a moving target! Parents try to help by holding the patient and distracting them with the latest smartphone that plays cartoons. Sometimes nurses rely on memory of where the vein would be located or the previous experiences of actually hitting the vein from feeling it bulge.

One innovation that our pediatric unit uses is the AccuVein device. It is a small hand-held instrument that uses a near infrared light that when it is held over the hand, foot, or arm of a child will allow you to visualize the vein hidden under the skin surface and seen on a display window of the device. If you need to use both your hands for the IV start, the AccuVein can be placed in the wheeled stand that has a goose neck attachment to allow the nurse to position the vein finder however it needs to be for best visualizing of the vein.

The AccuVein has shown to be more reliable in assisting the nurse with visualizing the vein patterns when used correctly. It reduces the amount of needle sticks on the patient (Kim, et al, 2017).
