Recruitment will occur from the participants of the CHN Fellowship program. The participants will be new Community Health Network/Community Physician Network employees (Fellows). The human resources department of Community Health Network recruited all participants. The human resources department at Community Health Network interviewed primary care Fellowship candidates at multiple clinical sites throughout Indiana during March and April 2021. The total number of participants were the result of how many employees were hired and agreed to participate in the Fellowship program. This first Fellowship cohort consists of four (4) Fellows, all of whom are nurse practitioners.
The administration of the consent will occur prior to the Fellow completing the online surveys. Implied consent is detailed and attached to the beginning portion of each survey; the Fellow and Preceptors will imply consent for this project by returning the completed survey. The consent was constructed from a template provided by Community Health Network, and is attached. All Fellows and Preceptors will have an opportunity to imply consent. Before the consent and survey are offered, there will be a brief explanation of the study via email by the co-PI.

All participants in the Fellowship program will be offered an opportunity to take the surveys. The surveys will be administered via the Purdue’s Qualtrics program, and the results will be collected by the PI Christine Stesney or Co-PI Dr. Amy Arthur. The initial baseline survey will include questions that the Fellow will answer to generate a unique identifier. This identifier will be used for the remaining surveys to compare the data and help protect anonymity. The Fellows will be able to access and take the surveys at their discretion. This project will use a combination of four short surveys that will be distributed to the Fellows using a service called Qualtrics through Purdue University. The intention of the study is to evaluate the new CHNw APP Fellowship program; the PI Christine Stesney will use the study as her final doctoral program project. The data will be shared with the DNP committee to assist with analysis. This DNP committee consists of professors Dr. Julian Gallegos, Dr. Nancy Edwards, Dr. Barbara Winningham and Dr. Amy Arthur. The survey results will be housed on the encrypted Qualtrics site for the project’s duration, which is expected to be conducted over the summer and fall of 2021.
The construction of the survey questions for confidence used the C-Scale by Susan E. Grundy. The author granted permission to use this tool, which is attached. Dr. Susan Grundy states the Cronbach’s alpha ranged from 0.93 to 0.94 for this tool, indicating high internal consistency (Grundy, 1993). There will be five confidence questions that will be answered using a Likert-type scale. The participant can choose a number from one to five. Answers of five being the highest level of confidence for each question. The participant’s final score on all five confidences questions can be from 1–25.
In addition, there will be three questions to assess competency. The Weitzman Institute created the competency assessment tools used to create this section of the survey. The Weitzman Institute granted permission to use this tool, which is also attached. The Fellow will self-assess their competency level on a Likert scale of one to five. The Fellow can choose from novice, advance beginner, competent, proficient, expert, or N/A. The score five is the highest competency. The first competency question consists of eight general assessment, diagnosing, treatment, and management questions. The second question is more specific, gives the Fellow 14 common medical conditions, and asks them to rate their competency in assessing, diagnosing, treating, and managing those specific conditions. The final competency question lists five common procedures done in primary care. The Fellow will rate their competency performing those specific procedures.
The four surveys given to the fellows use the same questions; the fifth survey rephrases the questions for the Fellow’s preceptor. There will be a total of five surveys administered during the Fellowship program. The preceptors will evaluate the Fellow’s competency and confidence. The Fellows and Preceptors give consent via implied consent.
The Fellows commenced employment with a week-long New Provider Orientation on May 17, 2021, and then the following Monday began the Fellowship, which is composed of 3 distinct components. The first component of the APP Fellowship begins an 8-week-long didactic e-learning component. This component is a 12-credit post-graduate Transition to Clinical Practice Certificate Program, developed in collaboration with Butler University. The administration of the second survey will be after the 8-week didactic period.
During weeks 10-21, the second component of the Fellowship, the Fellows will rotate through specialty clinical sites with various specialty preceptors. These sites include cardiology, behavioral health, endocrinology, ortho/spine, urgent care, pulmonary, neurology, ENT, pediatrics, women’s, and dermatology. The administration of the third survey will be after the clinical specialty rotation component.
The third and final component is the primary care preceptor orientation portion. This component will occur at the Fellow’s assigned permanent primary care practice location. There will be support provided by the assigned collaborating physician, APP preceptors, as well as a mentor who practices within Community, but not in the same clinical practice as the Fellow. The fourth and final survey will be given one month into this primary care preceptorship.
The preceptor survey will include questions that evaluate the preceptor’s perception of the fellow. Consent is given via implied consent, as detailed on the top of the survey; by returning the survey, the participant implies consent. This survey will ask the preceptor their perception of the Fellows confidence and competency using similar questions as were given to the Fellows; these questions are attached. We will attempt to protect anonymity during the preceptor survey, as well, by using the same un-identifier process that is used with the Fellows. The preceptor’s impression of confidence and competency will be included in the longitudinal comparison done for this DNP project. Descriptive statistics will most likely be used due to the small sample size.
